UCFS is looking for candidates who are passionate about making a difference in the lives of others! We are currently seeking a part-time (20hrs) Community Health Worker who will utilize personal experience and training to assist individuals, families, couples, and groups with engagement into accessing and utilizing community resources. Will work conjointly with St. Vin- cent de Paul Kitchen to assist those consumers to be able to take advantage of UCFS services.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provides peer mentoring support, information and guidance relevant to consumer needs (social skills, substance use recovery supports, primary care, training, mentoring, behavioral health and recreational activities, etc.)
• Collaborates with behavior health, primary care, dental and other UCFS services and clients to determine care plans related to basic needs; legal, medical, and insurance. Assists clients to meet basic needs via case management, skill building and coaching.
• Maintain familiarity with community resources and collaborate with state and local agencies and other community based supports Assists clients with access to program services, area resources.
• Participates in Case Reviews and supervision
• Develop client-centered care plan that supports client empowerment and improvement of overall health status
• Minimum of 60% travel may be required

Requirements:

• Minimum of Associates Degree, BA/BS preferred.
• Ability to provide a strengths based, client centered approach to the delivery of service
• Proficient with Microsoft Suite, Internet and e-mail
